Demonstrating that your injury had occurred as a result or in relation to your employment usually involves providing medical evidence, witness statements, and documentation of how the injury occurred.
Key medical evidence to support your claim may include:
- A certificate of capacity (COC) detailing your diagnosis, treatment plan, and capacity for work.
- Medical reports from treating doctors outlining the nature of the injury and its connection to your work.
- Specialist reports or referrals confirming that your employment was a significant contributing factor to the injury.
- Documentation of any treatment received, including prescriptions, therapy, or surgeries.
Who can provide relevant medical evidence?
- A GP, preferably your NTD
- A specialist, such as a surgeon, psychiatrist or dentist
An independent medical examination (IME) may be arranged to obtain a second opinion, after an initial COC has been provided by the NTD.
